Elementary Assessment
Student Teaching Practicum
Below is an exit slip that my elementary students completed after flag football lesson. This lesson focused on introducing the 4th and 5th graders to a basic offensive formation in flag football. At the conclusion of the lesson, students were asked to match the each football position with the appropriate statement at the bottom of the paper. The lesson plan for this assessment is also attached below.

Below are examples of student responses for a 5th grade open response question. This assessment was conducted during a baseball unit at Park Forest Elementary School in State College, PA. This assessment took very little time to complete and the students displayed a number of correct answers! All national and state standards for this lesson are listed on the question form.
Middle School Assessment
Below is a peer assessment form for 8th grade badminton. When completing this assessment, students will observe their classmates based on performance. This assessment includes an eight-point scoring scale; however, the results will not contribute the a student's overall grade. The instructor will give each student a grade based on his or her participation in the assessment lesson.

Below are examples of peer assessment forms that were completed by 8th grade students at Mount Nittany Middle School in State College, PA. For the badminton peer assessment, each student observed a partner playing badminton. As his or her partner played, the observing student completed the assessment form for each skill listed. Prior to conducting the assessment, I gave the class a demonstration on observing and completing the assessment form correctly. The observation sheet includes clear directions for completing the assessment and recording a total score.
High School Physical Education Assessment
Below is an example of an exit slip that 9th Grade students completed at the conclusion of a physical education lesson on resistance band workouts. During this lesson, students were introduced to a variety of resistance band exercises and practiced all exercises during a full body workout. Each station in the workout, provided the students with information on the target muscles for each exercise. On the exit slip, students were asked to list the target muscles for the two exercises on the page. The lesson plan and activity description are also attached below.

High School Health Assessment
Student Teaching Practicum
To the left, is an example of a "Tobacco-Free Society" poster that a group of 9th grade students developed during our Alcohol, Tobacco, and Other drugs Unit. This lesson focused on the effects of smoking, beyond the health effects of those who use tobacco. Students worked within groups to develop posters that included information on effects within categories such as financial, social, environmental, and the health effects on others (second hand smoke). Students then presented their posters to the class and shared their favorite findings! The students did an outstanding job with this assignment and I displayed all of the posters in the health classroom! Attached below are the directions and grading form for this assignment.

Throughout my practicum health experience, I was fortunate enough to teach a lesson on eating healthy in college to 11th grade students at State College Area High School. This specific lesson focused on the many barriers associated with poor eating habits in college and presented some of the strategies for overcoming these barriers. After a brief discussion and presentation of the material, I had students develop a poster that would be on display in a college dorm hallway. This poster was to inform college students of tips and strategies to help them eat healthy while at school. After creating the posters, I had students present their posters to the class and explain why they deciding to include specific strategies on their poster. The lesson plan and analytical rubric for the "poster creations" activity are attached below.

Below are two example posters that were created by students at State College Area High School. Both posters include four tips/strategies for eating healthy in college, display great color, and have creative slogans! All students in the class responded well to this activity and enjoyed presenting their posters as well!
Analysis of My Teaching

Through discussions with my cooperating teachers and self-reflection, I was able to identify my teaching strengths and weaknesses for each week, and adjust my instruction accordingly. This also allowed me to measure the effectiveness of each lesson that I taught!
